Optimizing Medication Use in the PALTC Setting
The issue of polypharmacy and inappropriate medication use in post-acute and long-term care (PALTC) has been an ongoing concern. The PALTC setting has one of the highest polypharmacy rates, which increases the risk of adverse events and drug interactions.
Since May 2021, the Post-Acute and Long-Term Care Medical Association (PALTmed) and the American Society of Consultant Pharmacists (ASCP) have been holding meetings and developing resources to help prescribers, pharmacists, DONs, CNOs, CMOs, and CEOs optimize medication use in the long-term care setting. These activities are key components of the Drive to Deprescribe (D2D) initiative, aimed at reducing polypharmacy and enhancing patient care.
