The Centers for Medicare & Medicaid Services (CMS) reminds all skilled nursing facilities (SNFs) to meet the upcoming August 1, 2025, deadline to revalidate their enrollment and report updated ownership and management details. This initiative is part of CMS’ broader push to strengthen transparency around who owns and operates SNFs.
Last fall, SNFs received notices from their Medicare Administrative Contractors (MACs) directing them to submit updated information through the revised Medicare Provider Enrollment Application (Form CMS-855A). Facilities are required to disclose comprehensive details about their ownership, managing employees, and related parties.
To provide public access to this information, CMS began releasing the collected data on its data.cms.gov portal in December 2024. These updates allow the public to analyze ownership patterns and affiliations across the nursing home sector. As part of this reporting, SNFs must identify their medical directors, whose names will now appear on the CMS data website. Personal details like Social Security numbers, birthdates, and contact information remain confidential and are not published.
