Introducing the PALTmed Fellowship Program
The Fellow of PALTmed (FPALTC) designation is a prestigious, peer-reviewed credential that publicly acknowledges an individual’s ongoing dedication to the field of post-acute and long-term care medicine.
FPALTC status is granted to individuals who have shown a deep professional commitment to PALTC and are actively engaged members of PALTmed. This honor recognizes their exceptional dedication and sustained contributions to both PALTmed and the field of PALTC medicine.
Your FPALTC designation remains valid as long as you maintain your PALTmed membership.
Application Process Opens: April 2025
Application Deadline: September 15, 2025 October 15, 2025 (Deadline Extended!!!)
Acceptance Notifications: December 17, 2025
Eligibility Requirements
All PALTmed members are eligible to apply for the PALTmed Fellowship Program (FPALTC designation) if they meet the following criteria:
- A minimum of seven years of clinical practice in PALTC medicine (post-training).
- Demonstrated involvement in clinical, administrative, research, and/or teaching activities within the field of PALTC.
- Active membership with PALTmed for at least four consecutive years.
- Ongoing engagement in PALTmed initiatives and activities (at least 30 engagement points within the past five years). Refer to the Application Components section below for details.
Application Components
- Fellow Personal Statement - in 500 words or less, share why you want to become a fellow and what working in PALTC means to you.
- CV/Resume (7 page maximum)
- Letter of recommendation from a PALTmed member in good standing (with at least three years of membership) endorsing the applicant's advancement to Fellow of PALTmed (FPALTC) status. The letter should highlight the applicant’s notable accomplishments or contributions that showcase their dedication to advancing the field of PALTC and/or their clinical expertise in PALTC medicine.
- Engagement Activities – provide a breakdown of how you earned the required 30 engagement points within the past five years through various PALTmed activities. See below for details.
Engagement Activities
- Attendance at PALTmed’s Annual Conference within the past five years (3 points/meeting)
- Completion of the Online Core Curriculum on Medical Direction (3 points)
- Engagement in PALTmed Educational Programs, such as Drive to Deprescribe, More of a Good Thing, webinars, and more within the past five years (1 point/program)
- Publication of Manuscripts in JAMDA or Caring for the Ages (2 points/article)
- Participation as a Presenter at a PALTmed educational event, including the Annual Conference, webinars, and other programs (3 points/presentation)
- Service as a PALTmed Volunteer (3 points/year of service)
- Active Chapter Involvement, including roles as a presenter, volunteer, or delegate (1 point/year of service or presentation)
Note: you do not need points in every section of the application to achieve the required 30 points.
