CMS has issued a reminder for enrolled skilled nursing facilities (SNFs) to collect and submit data on ownership, managerial, and related party information by May 1, 2025. This initiative is part of CMS' ongoing efforts to enhance transparency regarding SNF ownership and leadership.

In the fall of 2024, SNFs received a revalidation notice from their Medicare Administrative Contractor (MAC), instructing them to report the required data using the updated Medicare Provider Enrollment Application (Form CMS-855A).

Starting in December 2024, CMS began publishing the collected data on its data.cms.gov website. This publicly accessible information allows users to explore trends and relationships within nursing home ownership structures. SNFs are required to report their medical directors, whose names will be available on the CMS website. However, sensitive personal data, such as social security numbers, birthdates, and contact information, will not be included in the published records. Currently, CMS is only publishing the medical directors' first and last names along with the facility.
