The Centers for Medicare & Medicaid Services (CMS) will revalidate enrolled skilled nursing facilities (SNFs) from October–December 2024 to collect data on ownership, managerial (e.g. medical directors), and related party information. Medicare administrative contractors will be sending revalidation notices:
- One-third of SNFs will get notices in October
- Two-thirds will get notices in November or December
SNFs must report this data on the revised Medicare Provider Enrollment Application Form (CMS-855A), which can be done via the online PECOS system or through paper enrollment applications.
The revised CMS Form 855A now includes several additions on individuals’ relationships to the SNF, including if the person serves as the medical director, and their roles with policies or procedures, clinical consulting services, administrative services, or management services. The form will require individual identifying information such as:
- Social security number OR individual tax identification number (ITIN)
- Date of birth
CMS has said that no sensitive information will ever be published and noted they will be issuing further guidance on this in the near future.
PALTmed will update our member guidance on this and keep members updated when new guidance is released.
