PALTmed resolves to present the following resolution to the AMA: that the American Medical Association (AMA), in conjunction with PALTmed and other interested organizations, develop a proposed procedure to promote efficient data transfer to accompany discharged patients to and from acute, long term care and subacute care settings. Data to accompany patients must include a patient's recent history and physical exam from a referring hospital, including baseline cognitive and physical functioning, any recent surgical procedures, status of all acute medical problems and plan of care, a list of medications and their indications, appropriate diagnostic results, presence or absence of advance directives, the last three or four days of progress notes as well as consultations, and any necessary follow up by medical providers from the acute care setting; and

BE IT FURTHER RESOLVED that PALTmed convey this position to the AMA House of Delegates and to the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO); and

BE IT FURTHER RESOLVED that PALTmed should develop this proposed procedure with other interested organizations in the event that the resolution does not move forward through the AMA.
