THEREFORE BE IT RESOLVED, Post-Acute and Long-Term Care Medical Association (PALTmed) make it a priority to work to improve and standardize the flow of critical information between the long-term care continuum through collaboration with other stakeholder organizations, including the AMA.

AND BE IT FURTHER RESOLVED, PALTmed work with AMA and other stakeholder organizations in an effort to develop standardized transfer forms and to promote educational initiatives that optimize transfer of information across the spectrum of care.

AND BE IT FURTHER RESOLVED, PALTmed work with the AMA’s Physician Consortium for Performance Improvement (PCPI) to develop specific measures appropriate for transitions of care in the long-term care setting.

AND BE IT FURTHER RESOLVED, PALTmed ask AMA and other stakeholder organizations to work with PALTmed to develop educational initiatives and long-range projects to optimize the transfer of information across the spectrum of acute and long-term care.
