A New Chapter for Caring for the Ages

Since its inception, the Foundation for Post-Acute and Long-Term Care Medicine has stood as the philanthropic heart of the post-acute and long-term care community. Created by and for PALTC professionals, the Foundation has always focused on one central goal: ensuring that clinicians working in the post-acute and long-term care medical field receive the highest-quality support in serving their patient population, grounded in evidence, compassion, and clinical excellence. Today, that mission is entering a new and exciting chapter through its deepening commitment to Caring for the Ages and a renewed partnership with Post-Acute and Long-Term Care Medical Association (PALTmed).

Caring for the Ages, PALTmed’s flagship publication, has long been a vital resource for clinicians working in nursing facilities, assisted living communities, and other PALTC settings. It offers practical guidance, emerging research, policy updates, and clinical pearls that directly inform day-to-day practice. The Foundation’s dedication to this publication reflects shared belief that high-quality information, presented in an accessible and practice-focused way, is one of the most powerful tools for improving patient outcomes and supporting the clinicians who care for them.

The Foundation’s support for Caring for the Ages is not merely symbolic; it is strategic. By investing in this publication, the Foundation helps ensure that crucial content reaches physicians, advanced practice providers, medical directors, nurses, and other interdisciplinary team members who are often working under immense pressure and in resource-limited environments. Articles on best practices in medical direction, dementia care, infection prevention, quality improvement, and regulatory change can immediately shape how care is delivered to some of the most medically complex and vulnerable patients. In this way, the partnership transforms philanthropic generosity into real-world impact in exam rooms, hallways, and resident rooms nationwide.

The Foundation’s collaboration with PALTmed is also a natural extension of its broader philanthropic portfolio. For decades, the Foundation has funded workforce development, research, and education—most notably through the Futures Program, which has supported more than a thousand emerging PALTC clinicians, and through grantmaking that advances innovation in care models, quality improvement, and clinical research. PALTmed, as the professional home for PALTC clinicians, provides the infrastructure, community, and professional standards that help those same clinicians thrive. When the Foundation and PALTmed work in concert, education, research, and practice align in a powerful, mutually reinforcing way.

This renewed partnership extends beyond funding and branding; it is about integrating the mission. The Foundation’s investment in Caring for the Ages creates a platform to share stories of philanthropic impact—highlighting research projects, educational initiatives, leadership development, and Futures alumni whose work is transforming care in their communities. Likewise, PALTmed’s clinical and advocacy leadership helps inform the Foundation’s priorities, ensuring that its philanthropic investments are targeted to the most urgent and emerging needs in the field. Together, they create a virtuous cycle: philanthropy fuels innovation and education; education and innovation elevate practice; and improved practice drives new ideas and opportunities for future philanthropic support.

For donors and supporters of the Foundation, the partnership with PALTmed and Caring for the Ages offers a unique value proposition. Contributions to the Foundation not only support scholarships, research grants, and special initiatives; they also help to sustain a trusted, widely read publication that reaches clinicians where they are. Each issue of Caring for the Ages becomes both a clinical resource and a touchpoint for sharing the Foundation’s impact, reminding readers that behind every article, scholarship, and grant is a community of donors who believe in excellence in practice for every practitioner and vulnerable patient in their care. This visibility strengthens the culture of giving within the PALTC community and reinforces the idea that philanthropy is not an add-on—it is a core driver of progress.

Looking ahead, the Foundation for PALTC Medicine’s dedication to Caring for the Ages and its partnership with PALTmed position both organizations to meet the challenges of a rapidly changing healthcare landscape. As the population ages, medical complexity increases, and care settings evolve, the need for specialized knowledge in PALTC will only grow. Through this collaboration, the Foundation and PALTmed are building a stronger bridge between philanthropy and practice: one that delivers practical education, supports the workforce, advances research, and ultimately improves the lives of patients and residents in post-acute and long-term care.

In short, the Foundation’s commitment to Caring for the Ages and its close alignment with PALTmed reflect a shared vision: a world in which every PALTC clinician has the resources, training, and support they need to deliver outstanding care—and where philanthropy, education, and clinical excellence move forward together, hand in hand.

I hope that those reading this will continue to support the Foundation for PALTC Medicine with your intrigue, engagement, and philanthropy, as together we are building a brighter future for our PALTC field of medicine.
