The PALTmed Futures Program is a one-year learning experience introducing clinicians to opportunities in post-acute and long-term care (PALTC) medicine. 2027 marks our 26th Futures class. For more than 25 years, Futures has helped emerging clinicians explore the field, connect with PALTC leaders, and build relationships beyond the annual meeting.
- Geriatric medicine or geriatric psychiatry fellows
- PGY II–III internal medicine or family medicine residents
- Advanced practice providers, including nurse practitioners and physician assistants, including those new to post-acute and long-term care
- Physicians new to post-acute and long-term care
- Allied health professionals completing training or new to the setting
Previous Futures participants are not eligible.
What You'll Experience
01 | IN-PERSON LEARNING
Begin at PALTmed27
Start with an intensive, one-day Futures experience on March 18, held in conjunction with the annual meeting. Learn from experienced PALTC clinicians and leaders in Albuquerque, NM, March 18-20, 2027.
02 | CAREER EXPLORATION
Discover career opportunities
Explore different ways to practice, including medical direction, and gain practical insight into the clinical and professional realities of PALTC.
03 | PROFESSIONAL CONNECTIONS
Build your network
Connect with peers who share your interests and PALTC leaders you can turn to as you build your career. Develop relationships that extend beyond the meeting.
04 | CONTINUED INVOLVEMENT
Stay engaged all year
Futures Connect offers PALTC-focused resources and leader discussions. PALTmed membership includes the member forum, a JAMDA subscription, education discounts, and opportunities for continued learning and volunteer involvement.
How to Apply
Submit all four required materials with your online application. Use the checklist below to prepare.
Applications open in early November.
Need Travel Assistance?
Airfare and hotel assistance is available through PALTmed/Foundation. Applicants are encouraged to apply even if they need support. Indicate your needs in the application; specific funding and travel arrangements will be shared with accepted participants.
Meals provided as part of Futures are covered. Participants are responsible for other meals and transportation costs.

I heard about a conference in San Diego when I was a fellow. I honestly did not know much about what PALTmed did, but San Diego sounded lovely, so I applied for some program named “Futures” to facilitate offset my costs, and I’ve never looked back! It was transformative because once there, I felt I’d found my professional home and was intrigued by the prospect of focusing my career on long-term care. I met colleagues from Colorado. The day after I finished my fellowship, I started as the medical director of a struggling building, and the fun began! Fortunately, I always had PALTmed colleagues and resources to help guide me through those challenging waters. I’m grateful that at every step along my career path, PALTmed has been there to help me professionally and personally. Wonderful to know that all of this nurturing started with Futures!

I was thrilled to be a part of the program. I was the only social worker in the group, but I remember feeling welcomed by everyone! It was also my first time at the PALTC conference, and being a part of FUTURES helped me to feel connected in what otherwise would have been a very overwhelming experience. I am happy to report that I was awarded my first NIH grant as PI, an R21 entitled The Impact of Live Discharge from Hospice on Patients and Caregivers.
