The “Assisted Living Facility Medical Director’s Contract” template is designed to be a model agreement for Assisted Living Facilities (ALFs) who are interested in contracting with a physician to provide oversight of medical services. Given the increased levels of medical complexity that is becoming more common in residents of assisted living, a formal relationship between a physician and an ALF can be valuable. The COVID-19 pandemic highlighted the beneficial role that a medical director (or advisor or consultant) can play.
While much of the content of this model agreement can apply to larger ALFs, smaller facilities are encouraged to adapt this to suit their needs and resources. Facilities may also need to adapt it to comply with their specific state and local regulations. Since ALF’s are not subject to federal oversight or mandates, there are no “hard and fast” rules pertaining to ALF’s, and the significant variability across facilities related to size, medical acuity of residents, staffing levels, etc. necessitates flexibility around the role and responsibilities of any contracted physician.
