Recertify your CMD credential

Apply during the sixth year of your current certification period. Gather your credit documentation, then complete and submit your recertification application in the CMD Portal.

Types of credits that can be used

CMD Management credits: These credits focus on the administrative and leadership responsibilities of a medical director, including leadership, operations, compliance, and regulatory requirements. Qualifying topics may include quality improvement, survey readiness, staffing, and risk management.

CMD Clinical credits: These credits focus on direct patient care and clinical decision-making, including diagnosis, treatment, and evidence-based care. Qualifying topics may include dementia care, infection prevention, and medication management.

AMA PRA Category 1 Credits™ focused on post-acute and long-term care: These credits can be used toward CMD requirements only when the educational content focuses on post-acute and long-term care. Applicants must submit documentation verifying the educational activity and its content. Documentation may include a certificate of completion and course agenda that identify the activity, credits awarded, and relevant post-acute and long-term care content. Combine all supporting documentation and upload it as one document.

Before you begin...

Review the eligibility requirements below and gather any supporting documentation you may need.

1. Confirm your eligibility

You must hold a current CMD credential, an unrestricted state medical license, a current DEA license, and consent to a FACIS Level 3 background check.

2. Gather six years of CMD credit records

Document all required credits in the CMD Portal, including 60 CMD Clinical credits and 60 CMD Management credits earned during your current certification period.

3. Complete the recertification application and payment

Enter your information, upload the supporting documents, and pay the $400 member or $500 non-member fee in the same application.

4. Submit your application in the CMD Portal

You can save an incomplete application and return later. Make sure all supporting records are attached, then submit your recertification application in the CMD Portal.

If it lapsed more than six months ago, email certification@paltmed.org to request the reinstatement form.
